Eligibility and Referrals

People of any age, with any disease, may receive hospice care. Two main guidelines help determine when this care is appropriate:

• If the patient has a life expectancy of six months or less if the disease continues its normal course.
• If the patient chooses to change the focus of care from actively treating the disease to receiving comfort care.

Referrals: To make a referral to Hospice House of Mid-Michigan or Sparrow Hospice Services call 517.364.7200 or 888.636.8236.

A patient, a family member, a friend, physician, clergy or healthcare professional may refer persons to Sparrow Hospice Services.

Sparrow Hospice Services is committed to providing the same quality care to every patient regardless of their ability to pay. Insurance coverage varies from policy to policy. At times what is covered under one policy is not covered under another.
