Registration
If you are a new patient to Sparrow Family Health Center - Central (SFHC-C), you will be asked to give us basic
registration information. Please let us know if any important information such as your name, address, telephone
number, or insurance coverage changes while you are our patient. Be prepared to show your insurance card each
time you come in for an appointment.
If you are unable to make a scheduled visit, please call the office and
cancel your appointment, preferably 24 hours in advance.
Consultations
SFHC-C has a professional relationship with many medical specialists in the community. If you need the services
of a specialist, we will make a referral based on your medical needs and consideration of your insurance
requirements. It is usually necessary to see you first to determine which specialist would be best for you before
a referral is made. As your primary care doctor, SFHC-C will care for you in conjunction with the specialist.

To better serve you we have an automated telephone system. Please dial the main office number at 517.364.5710 and then select:
1 To schedule an appointment or to get a referral
2 To discuss billing issues
3 To discuss provider link or to discuss medical record issues
4 To get a prescription refilled
5 To have a nurse return your call or if you have a medical concern
6 To get telephone and office hours
9 To have the selections repeated
For Medical Emergencies call 911
We have a doctor on call for emergencies after hours. If you have a medical problem that cannot wait until the next
business day, you can contact the on-call doctor through the Hospital operator, 517.364.1000, and ask to speak to
the "Family Practice Resident on-call."
Same Day Appointments
Same day appointments are available if you have an illness or injury that requires immediate medical attention. If possible, you will
be seen by your primary medical provider. However, if your provider is not available, another provider may see you. We will do our
best to see you in a timely fashion.
Lab Results
You will be informed of your routine lab results by mail. Some of your results may take up to four weeks to receive and not
all results may arrive at the same time. Lab results of a sensitive nature or requiring further discussion will be addressed by a personal phone call or follow-up appointment.
Prescription Refills
We ask that you have your prescriptions refilled during your office appointments. In the event that it becomes necessary to have your prescriptions filled using the prescription phone line,
we will order enough medication to cover you until you are seen in the office. It is our policy to have all medications called in to a pharmacy within one business day. When using the prescription phone line, please say and spell your
name and the name of the medication. Include the dosage of the medication, the pharmacy name, the pharmacy phone number, and the phone number where you can be reached.
It is helpful to bring an up-to-date list and all of your medications with you for review during your office appointments.
For your safety, we limit prescribing new medications over the telephone.
As a matter of policy, our on-call physicians DO NOT call in prescriptions for narcotics or any other controlled substance.
